TEAM EVALUATION FORM

Team projects are graded based on a number of factors. A significant outcome of the team project is that you develop the critical capacity to operate effectively as a member of a team. This means that you participate in the development of team member expectations and that you personally deliver in a manner consistent with those. Twenty percent of your grade will be based on team peer review. The completion of this form will lead to the assignment of points related to this twenty percent. Consistently negative reports by teammates will result in a significant loss of credit with regard to this component.
On the following form, you are to evaluate each of your teammates based on "quality of his or her contributions" and "contribution effort and consistency."
Quality of contribution has to do with your assessment of the quality of the work completed by the individual. Was his or her final work product acceptable or did the team have to do "rework?" Contribution effort and consistency
has to do with his or her reliability and consistency. Did he or she attend class and, as a result, attend class time team meetings? Did he or she attend out-of-class team meetings? Did he or she accomplish all promised work products on a timely basis?
Please rate each teammate in each of these two areas. You should assign him or her a score of 1, 2, 3, 4, or 5 and make any comment/s that would explain your rating. A score of 1 suggests that the individual did not deliver in terms of expectations related to both quality and effort/consistency. A score of 3 is fair and suggests that performance was mixed. A score of 5 is excellent and suggest that the individual met or exceeded expectations in terms of both quality and effort/consistency.
